Home Health and Hospice care has become critically important as more and more care is shifted from inpatient settings to patients’ homes, and providers at all levels of care are both more integrated and held more accountable under at risk payer strategies in the context of shrinking resources.
This webinar will explore how the future healthcare landscape will shift patients with higher acuity and from acute care settings directly home, in many cases bypassing traditional “step down” providers. This trend will demand changes in practice, communication, information and risk sharing, leading many health systems to have a renewed understanding of and desire for close relationships with home health and hospice companies.
Home health and hospice providers, traditionally the eyes and ears of PCPs and specialists managing patient care at home, can become the glue that connects all facets of a well-integrated health system network. These providers can help answer the critical questions of “what care, in what frequency and in what setting” by taking into account patient condition, community supports, safety concerns and patients and families’ goals of care to help drive down over-utilization and keep patients at home, where they want to be.

Karen Gomes is the President and CEO of Home Health Foundation, a Wellforce Member. Ms. Gomes has over twenty years of experience as a Registered Nurse, and has worked in the home health and hospice industry for over fifteen years. In addition to her RN degree, she holds a Master’s Degree in Gerontology from the University of Massachusetts and is a Certified Professional in Health Care Quality. Using data to drive decision making is a cornerstone of her leadership style and was the subject of several national presentations at VNAA and NAHC Annual Meetings. Her innovative use of technology, including first in the nation use of neuro-robotic stroke technology in home care led to selection as a panel speaker at a national leadership summit in October, 2011. More recently, Ms. Gomes has led clinical workgroups focusing on the care of vulnerable populations, including those with complex, advanced illness. Under her leadership, Home Health Foundation has been involved in a multi-year initiative to improve the care of patients with COPD, and is an active participant in the IHI Conversation Project. Ms. Gomes strives to design clinical delivery systems that realize the vision set forth by CMS, to deliver “the right care to every patient, every time.”
Patricia Buiocchi is the Chief Operating Officer of Home Health Foundation, a Wellforce Member. With more than 25 years of clinical and operational expertise in a variety of post-acute settings, including skilled nursing facilities, outpatient centers, rehab hospitals and home health care, she brings extensive experience in the strategic development of a post-cute system. As a black belt in Lean Certification, she has established innovative program development and operational integration throughout her career to meet the needs of a complex health system.
